To all of the CTECS Athletes and Parents
There are major changes to the process of signing up for sports as required by the new state laws that took place as of 6/1/2015.
Parents must perform the following tasks
(1) Take the on-line concussion quiz on the district/school website and select “parent/guardian option”
(2) Complete form and sign-off on the concussion awareness form.
(3) Complete form and sign-off on the sudden cardiac arrest form
(4) Complete and sign-off Athletic Participation Form.
Students must perform the following tasks
(1) Take the on-line concussion quiz on the school website and select “student option”. Concussion Training Link
(2) Complete form and sign-off on – CIAC Sudden Cardiac Consent Form 2019-20
(3) Complete form and sign-off on – Concussion Parent Consent 2019-20
(4) Complete and sign-off Athletic Participation Form.
Before a student athlete can practice they must have a sports physical on file with the nurse, the above forms turned in to their coach and student and parents must have taken the on-line concussion quiz.
